A geriatric Care manager works with families to identify, plan, and oversee the care of their aging or disabled relatives. They meet with families and clients to determine their needs, find ways to meet them and manage care plans to make sure they are effective. A geriatric Care manager's job responsibilities include:

•       Meeting with families and recommending services

•       Determining which services clients are eligible for

•       Presenting housing options

•       Assigning and recommending caregiver Agencies

•       Scheduling services

•       Finding social activities clients can participate in

•       Helping families with the financial and legal aspects of their relatives' care

•       Visiting clients to assess their care and needs

•       Creating short- and long-term plans for client care

•       Updating families regularly

Geriatric Care managers also help guide clients' families during emotional and stressful periods and help them make difficult decisions. They work closely with nurses, doctors, healthcare staff, insurance agents and families. They spend time working in an office but travel to meet with clients.
